As a passport acceptance agent, I can tell you that probably 90% of the photos we see are pretty bad. I usually tell people, No, really, you want your picture to look like you've just stumbled off of a 12-hour flight.
And yes, you're right about the smiling: one of the most recent passport newsletters emphasized having a "neutral" expression, and avoiding excessive smileyness.
